Primary Health Care - An age-friendly approach

Access to good primary health care promotes active ageing by enabling older people to age and stay as healthy, and as long as possible in their own homes and communities. It prevents premature nursing home placement, and unnecessary costly hospitalizations.

Primary health care (PHC) services are usually the older patients’ first point of contact with the health care system before they are referred to acute hospitals or specialists for treatment of critical illnesses.
The primary health care physician, usually a private general practitioner or a physician in a government polyclinic, plays an important role in detecting chronic diseases early, promoting and maintaining the health of patient.

  • Hua Mei Acupunture and TCM Centre - alternative TCM and acupuncture services for pain management and health maintenance
  • Hua Mei Care Management - care management for those with complex medical problems and socio-financial needs.
