AN OLDER person with a long-term medical condition is discharged from the hospital and recuperates at home. Family members visit the patient but are either too busy to provide daily care or just do not know how. This is where geriatric care managers provide invaluable help.

Our Care Managers at the Hua Mei Care Management Service (HMCM) form an important bridge between the frail older person who has high medical risk and low social support, and community health resources.

They help frail, ambulant or homebound elderly navigate the complex public and community health and social services to find the right service, and at the right cost to manage their health conditions.

They visit the older patient at home and conduct an assessment of the patient’s medical issues, functional and financial abilities, home safety, formal and informal support systems, and then make recommendations for appropriate services.

Our care managers will explore how community resources such as mobile clinical services, rehabilitation, day care, financial assistance, home help and home modifications can support the older persons’ desire to continue living in their own homes and community.

Our service is funded by the National Council of Social Service (NCSS) and Ministry of Community Development, Youth and Sports (MCYS), and is aimed at maintaining the integrity of the elderly and family relationship by facilitating choices.

Through care management, older people and caregivers can be empowered with knowledge and means to make informed choices in the planning and determination of care.

Who is eligible?

1. Frail older persons who are at least 60 years old.
2. Those who have at least one chronic illness (e.g. diabetes, hypertension) requiring medical follow-up together with at least three social services need (e.g. financial assistance, transport and escort)
3. Elderly who reside in the western Region of Singapore in the catchment area stretching from Alexandra to Clementi.

What do Hua Mei care managers help?

Following a full assessment of the older patients’ medical, social and financial needs, our team of nurse and social worker care managers will propose a care plan, in consultation with them and their caregivers.

Our care managers will explore how community resources such as home care, home help, rehabilitation, day care, financial assistance, and home modifications can support the older persons’ desire to continue living in their own homes and communities.

How do I make a referral?

We accept referrals from: hospitals, doctors, Voluntary Welfare Organisations, friends, families, neighbours, volunteers, community-based organisations, self-referral.
